CASE MANAGEMENT/INTERVENTION

On a daily basis the social service staff will be responsible to report on client individually and a file is maintained under lock and key. Daily documenting progress and incidents, written results of consumer’s assigned task will be reported and tracking of client participation with the program. Staff will schedule a weekly calendar of Individual, Group, and Family counseling sessions with residents and outreach youth Monday- Friday.
