INTERVENTION
The
first counseling sessions is conducted as an intake
interview. At intake it is very important for agency
staff to receive two items of identification, Social
security card, Georgia I.D, or birth certificate. At
this informational gathering session other
interventions will be scheduled by staff. The
consumer assessment session is completed by a
licensed therapist on our staff and is conducted
within 48 hours of enrollment. In the Intake session
consumer and or/legal guardian is required to sign a
permission slip to received individual, group and
family counseling while enrolled in the program.
Each consumer will be required to obtain up to two
(2) sessions each month. The staff conducts a bi-
monthly management review of all consumers on its
caseload. This session is identified as a bi-weekly
Staffing. In this session all consumers enrolled in
the agency’s outreach and residential programs will
be reviewed and progress documented in files. The
review session has a paper trail documented as the
Case Management Plan and Review. This collective
information will identify the days lodged, workshops
attended, meals served, any problems or positive
progress the consumer has made during a 14 day
period. Since major emphasis is place on
independent training techniques the three step
program training levels are mention and staff
document progress reports toward the three step
matriculation process levels of observation,
demonstration and independent living.
